Include the following information in the "your personalization" box

- Personalization Top - Breed
- Personalization Bottom - Pet Name
- Add 8-12 custom words, short phrases that describe your dog's characteristics or funny sayings you say to your pet
- Font as shown or custom font choice
- Proof Available Upon Request
